<div class="WordSection1"><p><strong>Clinical Pharmacist needed for growing Community Health Center in Phoenix.<br /></strong></p><p><strong>JOB SUMMARY</strong></p><p>As a member of the integrated health care team the Clinical pharmacist provides specialized pharmaceutical services to patients and providers to improve drug usage and therapeutic outcomes.</p><p>The position facilitates<strong><em> Medication management</em></strong> through medication reconciliation; evaluation of adherence, effectiveness, toxicity and is a resource to providers and members of the care team on issues concerning drug therapy, toxicity of drugs and side effects as well as assisting in appropriate doses.</p><p>Responsibilities also include coordinating with other pharmacy technicians and pharmacists and participation in provider and pharmacy student&rsquo;s education.</p><p><strong>ESSENTIAL DUTIES AND RESPONSIBILITIES</strong></p><p>-Conduct and evaluate medication histories, assess compliance and suggest modifications for optimal drug product selection in order to achieve desired outcomes; instruct patients in correct use of prescribed medications</p><p>-Individualize medication regimens using sound principles and understanding of drug absorption, distribution, metabolism and elimination to establish and improve to optimum patient benefit</p><p>- Prevent, detect and resolve medication-related problems. Make evidence-based, patient-centered drug therapy recommendations as part of the Integrated Care Team</p><p>- Effectively verify patient medication orders for appropriateness and accuracy. Provide medication reconciliation and evaluate medication appropriateness and completeness with respect to patient data, including age, height, weight, diagnosis, and other pertinent patient information. </p><p>- Utilize clinical pharmacy skills, including contraindication and duplicate therapy checks, in assessing and formulating recommendations to physicians ensuring quality and economical drug therapy.</p><p>- Perform clinical rounds and chart reviews to facilitate pro-active problem solving, productivity and teamwork</p><p>- Provide concise, applicable, comprehensive and timely responses about medication queries from patients and providers</p><p>- Actively participate in development and implementation of related performance/quality improvement and patient safety activities. Report medication errors and adverse reactions. Participate in clinical guideline development, disease management and drug use programs</p><p>- Engage in professional self-development. Contribute to the training and proficiencies of pharmacy personnel, including pharmacy students.</p><p>- Support the health care team&rsquo;s efforts to provide patient centered care as part of the integrated health care team, educate patients on health maintenance activities and facilitate referral to other integrated services such as behavioral health and dieticia</p><p>- Other duties as assigned</p></div><p><strong>QUALIFICATIONS</strong></p><p><strong>EDUCATION and/or EXPERIENCE</strong></p><p>Graduate of an accredited School of Pharmacy. </p><p>Six months experience in a clinical setting or completion of an accredited clinical residency program.</p><p><strong>CERTIFICATES, LICENSES, REGISTRATIONS</strong></p><p>Current Arizona License or eligibility for license required, and Current CPR certificate</p><p><strong>PREFERRED</strong></p><p>Certified Diabetes Educator (CDE)</p><p>Residency</p><p>Board Certified Pharmacotherapy Specialist (BCPS)</p><p>Demonstrated ability to achieve positive patient outcomes in a similar environment.</p><p> </p>
